Terms of Use

These Terms of Use ("Terms") are a binding agreement between you and TeamLife Health Group ("TeamLife Health Group," "we," "us," or "our") for your use of the TLC CareNow client portal at app.tlccarenow.com (the "Service"). The Service is offered only to residents and clients of participating senior living and care communities that have authorized access. By creating an account, logging in, or using the Service, you agree to these Terms and to our Privacy Policy. If you do not agree, do not use the Service.

1. The Service

The Service is a web-based portal that helps you request, schedule, and pay for non-medical and community care services arranged through your participating community and TeamLife Health Group. Features may include account registration, profile and care preferences, booking and cart checkout, payment method storage, visit history, messaging or notifications related to your care, and account management.

The Service is not a substitute for emergency services, medical diagnosis, or treatment. In a medical emergency, call 911 or your local emergency number immediately. Do not rely on the Service for urgent or life-threatening situations.

Information in the Service is for coordination and scheduling. Care professionals and your community determine what services can be delivered. We do not guarantee that any request will be accepted, staffed, or completed.

2. Eligibility and accounts

You must be invited or authorized by a participating community to use the Service. You must provide accurate, current information about yourself and your care needs, and keep your profile updated.

You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify your community care team and us immediately if you suspect unauthorized access or if your device is lost or stolen.

We may suspend or terminate access if we reasonably believe your account is misused, fraudulent, abusive, or poses a risk to clients, staff, or the security of the Service.

You must be at least 18 years old, or the age of majority in your state, to create an account. If you use the Service on behalf of another person, you represent that you have legal authority to do so and to bind them to these Terms.

3. Care services and separate agreements

Scheduling, delivery, quality, cancellations, refunds, and billing for in-person care are governed by the agreements between you, your participating community, and TeamLife Health Group, including any community handbook, care plan, or service order you sign—not by these Terms alone.

A booking or payment through the Service does not by itself confirm that a visit will occur. Confirmation may require community approval, care-plan review, staff availability, and successful payment processing.

Fees, taxes, tips, and payment timing are disclosed during booking and checkout. You authorize us and our payment partners to charge your selected payment method for approved services.

4. Acceptable use

You agree to use the Service only for lawful purposes and in line with these Terms. You will not: (a) provide false or misleading information; (b) access another user's account without permission; (c) interfere with or disrupt the Service; (d) scrape, reverse engineer, or attempt to bypass security; (e) upload malware or harmful code; or (f) use the Service to harass, threaten, or harm others.

We may monitor use of the Service to protect security, prevent fraud, and comply with law. We may remove content or restrict access that violates these Terms.

5. Communications

By providing a phone number or email, you consent to receive Service-related communications, including one-time passwords for login, booking confirmations, reminders, and account notices. Message and data rates may apply. You may contact support to update contact preferences where available.

Marketing messages are not sent through the Service unless you separately opt in where offered.

6. Privacy

Our Privacy Policy describes how we collect, use, share, and protect personal information, including health-related information you provide for care coordination. The Privacy Policy is incorporated into these Terms by reference.

7. Intellectual property

The Service, including software, design, logos, and content we provide, is owned by TeamLife Health Group or its licensors and is protected by intellectual property laws. We grant you a limited, non-exclusive, non-transferable license to use the Service for personal, non-commercial care coordination while your account is active.

You may not copy, modify, distribute, or create derivative works from the Service except as allowed by law or with our written permission.

8. Third-party services

The Service may integrate with third parties such as payment processors (for example, Stripe), hosting providers, and authentication services. Your use of those services may be subject to their terms and privacy policies. We are not responsible for third-party sites or services linked from the Service.

9. Disclaimers

THE SERVICE IS PROVIDED "AS IS" AND "AS AVAILABLE." TO THE FULLEST EXTENT PERMITTED BY LAW, TEAMLIFE HEALTH GROUP AND ITS AFFILIATES, OFFICERS, EMPLOYEES, AND AGENTS DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.

We do not warrant that the Service will be uninterrupted, error-free, or secure. You use the Service at your own risk.

10. Limitation of liability

TO THE FULLEST EXTENT PERMITTED BY LAW, TEAMLIFE HEALTH GROUP AND ITS AFFILIATES WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR FOR LOST PROFITS, DATA, OR GOODWILL, ARISING FROM YOUR USE OF THE SERVICE.

OUR TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF THESE TERMS OR THE SERVICE WILL NOT EXCEED THE GREATER OF (A) THE AMOUNTS YOU PAID TO US THROUGH THE SERVICE IN THE TWELVE (12) MONTHS BEFORE THE CLAIM, OR (B) ONE HUNDRED U.S. DOLLARS ($100).

Some jurisdictions do not allow certain limitations; in those cases, our liability is limited to the maximum permitted by law.

11. Indemnification

You agree to defend, indemnify, and hold harmless TeamLife Health Group and its affiliates, officers, employees, and agents from claims, damages, losses, and expenses (including reasonable attorneys' fees) arising from your misuse of the Service, your violation of these Terms, or your violation of others' rights.

12. Changes and termination

We may update these Terms from time to time. We will post the revised Terms with a new "Last updated" date. Material changes may also be communicated through the Service or by email where appropriate. Continued use after the effective date of changes means you accept the revised Terms.

You may stop using the Service at any time. We may suspend or discontinue the Service or your access with or without notice where permitted by law or your community agreements.

13. Governing law and disputes

These Terms are governed by the laws of the United States and the state in which TeamLife Health Group maintains its principal place of business, without regard to conflict-of-law rules.

Before filing a claim, you agree to contact us and attempt to resolve the dispute informally. Except where prohibited by law, you agree that disputes will be brought in the state or federal courts located in that same state, and you consent to personal jurisdiction there.

If you are a consumer with rights that cannot be waived under applicable law, those rights remain unaffected.
